Jim Rivers Senior Cotton States Invitational Golf Tournament

Entry Form

The field will be limited to 100 players. Entry acceptance will be confirmed by mail or email with payment instructions. There is a $400.00 fee that includes green fee, cart fee, and range balls for the tournament and practice round. It also includes bag storage, club cleaning, casual party, lunch, awards luncheon, prizes and gifts. Breakfast is available but is not included in the entry fee. If your wife or guest is accompanying you, please include $75.00 additional. No partial payments please. The deadline is April 20th.

The Tournament Committee reserves the right to invite players of national & regional reputation, as well as other accomplished players, to play. Receipt of an application does not in itself constitute an invitation to The Jim Rivers Senior Invitational. The Tournament Committee reserves the Right to accept or reject any player or applicant at anytime. Committee Decisions are final.

In order for your entry form to be considered, this application must Be completed and received by the Tournament Committee 30 days prior to The start of the tournament. You will be notified as to your status by mail Or email on or before April 20, 2012.
